Farm & Home Supply 65th Anniversary Truck Giveaway Official Rules

1) No purchase is necessary to enter. Making a purchase at any Farm & Home Supply location will not improve an entrant’s chances of winning. To enter the giveaway, visit any Farm & Home Supply store between June 26 and June 29, 2025, to scan the official entry QR code and complete the online form, to be submitted between 12:01 a.m. (CT) on June 26, 2025, and 11:59 p.m. (CT) on June 29, 2025. Alternatively, you may pick up a mail-in entry form at the store during those same dates and submit it by mailing to: Farm & Home 65th Anniversary Truck Giveaway, Attn: Marketing, 4625 Broadway, Quincy, IL 62305. Mail-in entries must be postmarked by June 30, 2025. Limit one entry per person.
2) Entrants must be 18 years or older. Current Farm & Home Supply employees and their immediate families including spouse, children, or parents who reside with the employee are not eligible to win. Entrants must reside within a 45-mile radius of one of Farm & Home Supply’s 15 store locations (a list of locations can be found at

3) Description of the prize: One (1) 2025 GMC Sierra 1500 Elevation (MSRP Value: $62,920) (the “Truck” or “Prize”). The Truck’s mileage may increase throughout the Giveaway Period. The winner of the giveaway will be fully informed as to the Truck’s mileage at the time of winning.
4) One (1) winner will be selected by random drawing to be held on July 15, 2025. All valid entries, including both online submissions and mail-in entries, will be compiled into a master list. Each entry will be assigned a unique number, and a random number generator will be used to select the winning entry. The winner will be notified via telephone by the Farm & Home Supply Director of Marketing by July 16, 2025. The winner must accept the Prize by responding via phone by 11:59 p.m. (CT) on July 22, 2025. If the chosen winner cannot be contacted or does not respond by that time, the Prize will be forfeited and an alternate winner will be selected using the same method. The Prize will not be shipped. The winner must pick up the Prize at the Quincy Farm & Home Supply location, 4625 Broadway St, Quincy, IL 62305, on or before July 31, 2025.If the Prize is not picked up by July 31, 2025, at 8:00 p.m. another winner will be selected.
5) The odds of winning depend on the total number of eligible entries received. Many will enter, only one person will win. The Prize is guaranteed to be awarded and cannot be redeemed for cash or substituted, except at Farm & Home Supply’s sole discretion. Farm & Home Supply reserves the right to substitute the Prize with one of equal or greater value if necessary.
6) Any federal, state, and local taxes (including income and sales taxes) and any other costs or expenses associated with the acceptance and use of the Prize are the sole responsibility of the winner. The winner of a Prize will be required to complete and return a W-9 form before receiving the Prize, and the fair market value of the Prize will be reported on an IRS Form 1099. The winner will be required to pay for tax, title, registration and other fees when applicable.
a) If the winner is an Illinois resident, the winner is responsible for reporting the transfer of title to the Illinois Department of Revenue (Form RUT-50) and is solely responsible for any Use Tax owed, and any other fees owed to the Illinois Secretary of State as a result of receiving the Prize. Farm & Home Supply expects the Use Tax for the Truck to be approximately $2,600. This does not include income tax as reported by Form 1099. The standard fee for title transfer in Illinois is $165.00
b) If the winner is a Missouri resident, they will be required to pay the title transfer fee ($8.50), title processing fee ($6.00), sales tax calculated at 4.225%. The winner will be responsible for all reporting requirements to the state of Missouri and Missouri Department of Revenue, including, but not limited to, a Certificate of Title, Application for Missouri Title and License (Form 108), an identification number and odometer inspection, and other documents as applicable. The winner may be responsible for additional fees depending on the county they live in.
c) If the winner is an Iowa resident, they will be responsible for registering the Prize in the treasurer’s office of the county they live, and for obtaining an Iowa title by completing the following: Signed title from previous owner/dealer, Sales tax receipt, bill of sale, or purchase agreement, Completed Application for Iowa certificate of Title (form 411007), Cancellation of Security Interest (form 411168), if an uncancelled Lien is showing on front of the title, Odometer Statement completed on back of title.
7) The winner agrees to allow the use of their name in advertising at the discretion of Farm & Home Supply. By receipt of any prize, winners agree to release and hold harmless Farm & Home Supply and their respective subsidiaries, affiliates, suppliers, distributors, advertising/promotion agencies, and prize suppliers, and each of their respective parent companies and each such company’s officers, directors, employees and agents (collectively, the “Released Parties”) from and against any claim or cause of action, including, but not limited to, personal injury, death, or damage to or loss of property, arising out of participation in the giveaway or receipt or use or misuse of any prize.
8) Limitations of Liability: The Released Parties are not responsible for: (1) any incorrect or inaccurate information, whether caused by entrants, printing errors or by any of the equipment or programming associated with or utilized in the giveaway; (2) technical failures of any kind, including, but not limited to malfunctions, interruptions, or disconnections in phone lines or network hardware or software; (3) unauthorized human intervention in any part of the entry process or the giveaway; (4) technical or human error which may occur in the administration of the giveaway or the processing of entries; (5) late, lost, undeliverable, damaged or stolen mail; or (6) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant’s participation in the giveaway or receipt or use or misuse of any prize. If for any reason an entrant's entry is confirmed to have been erroneously deleted, lost, or otherwise destroyed or corrupted, entrant’s sole remedy is another entry in the giveaway, if it is possible. If the giveaway, or any part of it, is discontinued for any reason, Farm & Home Supply, in its sole discretion, may elect to hold a random drawing from among all eligible entries received up to the date of discontinuance for any or all of the prizes offered herein. No more than the stated number of prizes will be awarded. In the event that production, technical, seeding, programming or any other reasons cause more than the stated number of prizes as set forth in these Official Rules to be available and/or claimed, Farm & Home reserves the right to award only the stated number of prizes by a random drawing among all legitimate, un-awarded, eligible prize claims. Other restrictions may apply.
9) Any disputes arising out of this giveaway shall be resolved individually, without resort to any form of class action, and exclusively by binding arbitration under the rules of the American Arbitration Association.
